COVID-19 Procedural Changes
01 Consent Form
Each patient or parent will need to sign a Consent to Orthodontic Treatment in the Era of COVID-19 just once during treatment.
02 Health Questionnaire
Each patient or parent will need to complete a short Health Questionnaire right before every appointment!
03 Patient Check In
Upon arrival you will be greeted by one of our friendly orthodontic concierges right at the office entrance.
Concierge Services at Check In
Questionnaire
First, our concierge will confirm that you've completed your questionnaire. She'll also confirm that we have your correct cell number and email address. Lots of information is passing electronically these days ...
Social Distancing
Next, she will request that only the patient enters the office during the appointment. When that's not possible (e.g. young children), she'll ask that only one parent or companion enter when the patient is ready to be seen.
Masks
Please understand that all parents and companions must wear a mask from home. We didn't make that part up. Please click the link below to watch a YouTube video that demonstrates how to make a homemade mask. It's easy!
Temperature Check
Next step ... Our concierge will take the patient's temperature (and parent if entering) with a no-touch forehead thermometer. If it's above 99.6 we'll have to reschedule :(
Back to the Car
The safest place to wait is where there aren't any strangers, so you'll be asked to wait in your car until we're ready to seat you. We'll call you on the cell number you gave us at check in to let you know when to come back in.
Appointment Time

Please call (731) 668-8922 or (800) 548-5303 ASAP to schedule the next appointment. You'll get a text and/or email regarding the next appointment rather than a paper slip.
